Our dedicated and professional Case Management staff is committed to providing high quality services to Individuals with developmental disabilities.
They have experience with non-waiver Developmental Disability services as well as the Community Alternative Program (CAP) waiver services and the time sensitive processes of this funding program.
If selected to provide your case management services, a case manager will explain and assist you with each process: diagnostic and service assessments, planning, monitoring and reassessment.
Once the first step, the diagnostic assessment, has been completed, submitted, and approved, the case manager will partner with you, your family, your friends and community providers to develop a plan to meet your needs and preferences for a successful community life.
You will be the guide and the focus at your Treatment Team Meeting to plan for your future and your life’s goals to:
* develop self-reliance
* exercise freedom of control
* achieve personal fulfillment
* establish and maintain relationships, and
* participate in community life
After this thoughtful and thorough planning process, the case manager will write your plan, review it with you and obtain your permission and signature to submit it for approval.
Once approved, the services that you want, that best suit your needs, with the provider (s) of your choice can begin.
Your case manager will then monitor your services on a monthly basis to evaluate effectiveness, ensuring that they meet your needs to your satisfaction. If your needs change, you, your case manager and/or a member of your treatment team can request a meeting to revise your plan.
