Community Outreach (CAP Services) is designed to provide quality care to children and adults with developmental disabilities who are at risk of becoming institutionalized. Person-centered, one-on-one support is provided in a variety of locations including the individual's home, foster home, care provider’s home or community setting. Community Outreach strives to promote the highest level of independence for each individual served. CAP (Community Alternatives Program) funded services include Supported Living, AFL (Foster Care Homes), and Respite, Day Habilitation, and MR Personal Care Services.
