RSS's Case Management staff is committed to providing high-quality services to individuals with developmental disabilities.  They have experience with non-waiver Developmental Disability services as well as the Community Alternative Program (CAP) waiver services.

Case Management helps individuals define their needs, strengths and preferences for a more self-directed and self-reliant life in their community.  The case manager explains and assists with each step of the process, including diagnostic and service assessments, planning, monitoring and reassessment. 

Person-Centered Plans are developed for each participant based on interviews and information obtained through assessments and evaluations.  The individual receiving services is the guide and focus of the Person-Centered Planning meetings.

The Person-Centered Plan is the road map used to connect each individual to formal and natural supports that will enable him or her to attain goals and live a safe, healthy and successful life.  Once this plan is approved, the individual can begin receiving the services that he/she wants with the provider of his/her choice.

The case manager will monitor those services on a monthly basis to evaluate effectiveness, ensuring that they meet the individual's needs. If the needs change, the individual, the individual's case manager, and/or a member of the treatment team can request a meeting to revise the plan.
